The Home from Hospital Service aims to address two key issues that many NHS Trusts face – how to avoid delays in the discharge of patients from hospital and how to reduce avoidable readmissions to the emergency department.
Our evaluation of the service will focus on the Guys and St Thomas’ Hospital NHS Trust. The Home from Hospital Service was developed and operated by the British Red Cross, and commissioned by the Trust to be delivered at St Thomas’ Hospital since 2014. The service assists around 500 patients a year.
The Service works with hospital staff to identify patients that are most likely to be involved in delayed discharge and readmission to the emergency department, and then provides a 4-6 week package of tailored support for these patients, in their home.
M·E·L Research has been tasked with carrying out a retrospective evaluation of the service, from the perspective of patients, carers, staff and stakeholders in the Trust, as well as local authorities and Third Sector organisations.
We will assess service delivery and design as well as looking at overall service effectiveness, with the findings being used to inform ongoing practice and potential roll-out to other hospitals.
